 The District is committed to transparency and compliance with Oregon Public Records Law (ORS Chapter 192). To ensure requests are handled  consistently, efficiently, and in accordance with the law, all public records requests must be submitted using the District's Public Records Request form. Requests are reviewed to determine whether the records are disclosable, partially  exempt, or exempt under state law. The District will acknowledge requests within the timeframes required by law and will provide an estimated timeline and any applicable fees before fulfilling trhe request. Records are provided in the manner required by law and may be subject to redaction to protect confidential or exempt information. 

 Fairview Water District is pleased to offer annual backflow assembly testing directly to District customers. Testing will be performed by a certified Backflow Assembly Tester. Use the form below to authorize the District to test your assembly. 

Participation in the District program is optional. Customers may continue to hire another properly qualified backflow assembly tester.
